- At Health Connect America, we strive to provide timely and efficient access to our services. To initiate a referral, please contact our office and provide the following information to the office manager: Referral source name and contact information Client's full name, date of birth, and social security number Client/guardians home address and contact number Insurance information (if applicable) Reason for referral Once the referral is received, it will be processed within 24 hours. A counselor will then follow up to schedule an intake assessment. Appointment availability will be within 72 hours of the first successful contact with the client/guardian or at a time most convenient for them.
