Prescription Medication Policy Details
  • All prescription and over-the-counter medications must be reviewed and approved by medical staff at admission. Medication-Assisted Treatment (MAT), including buprenorphine and naltrexone, is available and monitored by licensed providers. Referral / Admission

Referral process
  • Clients, loved ones, or providers begin by completing the online "Get Started" form on the Provive Wellness website (selecting options like “Looking for services for myself,” “Referring a loved one,” or “A provider referring a patient”). Once submitted, the admissions team follows up directly to schedule consultations and determine appropriate levels of care. For questions or immediate assistance, referrals can also be initiated by calling (615) 640‑9994, the dedicated admissions line
Admission criteria
  • Provive Wellness is a dual-licensed outpatient clinic that treats individuals with mental health, substance use, or co-occurring conditions, including services for both adults and adolescents . They accept clients into Partial Hospitalization (PHP), Intensive Outpatient (IOP), and Outpatient (OP) programs. Admission is based on a clinical assessment to evaluate the individual's presenting issues (e.g., addiction, trauma, depression, anxiety), appropriateness for care level, and treatment needs. Coverage is available for those with insurance or those paying privately; financial options include self-pay and out-of-network plans
