About

The Revive Outpatient Center will assist persons in developing and maintaining coping skills that facilitate adjustment and integration within their living environment and community. Clients and family members will be empowered to make decisions about their care with the expected outcome of an increased quality of life. By staying true to our core values of dynamic recovery, inward journey, restored lives, and life-long wholeness, we aim to create a plan that is right for the individual, ensuring they have the access to the type of treatment they need most including the following: Six weeks of Intensive Outpatient Program. This may or may not include Medication Assisted Treatment(MAT) depending on the individual’s needs. Program services include group and individual clinical sessions, Peer Support, 12 step meetings and Targeted Case Management services. Phase 1 of Outpatient Program from 6 weeks to 4 months. This includes continued MAT if needed, individual clinical sessions, continued Peer Support and 12 step meetings, and Targeted Case Management. Phase 2 of our Outpatient Program from 4+ months. Services are tapered off as the client continues to work towards sobriety and as they create and sustain relationships, employment, and dynamic recovery. Personal sponsors for Peer Support are required until month 6, 12 step meetings are still required, and licensed clinicians and Targeted Case Management services will be available if needed. What is Medication Assisted Treatment (MAT)? Per SAMHSA, Medication Assisted Treatment (MAT) is the use of FDA approved medications in combination with evidence-based behavioral therapies to provide a whole-patient approach to treating Substance Use Disorders (SUDs).

Policies

Referral / Admission

  • Admission criteria
    • 1. ASAM criteria is followed, and the client must meet 2.1 level of care criteria for a substance use disorder. 2. Opioid or alcohol dependency diagnosis based on the most recent DSM criteria for diagnosis. 3. Recent opioid/buprenorphine use within the last 30 days. 4. Drug test screening results do not present complicating factors (for example- multiple positive drugs, benzos). 5. The client’s medical evaluation does not find any condition that would contraindicate his or her ability to participate in MAT services. 6. The client agrees to fully participate in all program components.
