Referral / Admission

  • Referral process
    • Intake Screening Policy It is the policy of All Together Recovery that the Intake Procedure is utilized to determine the appropriateness of admission. The organization will only provide admission for individuals who have needs that are appropriate for the level of care and treatment services provided by ATR. The intake screening process is conducted by Admissions Staff that have been appropriately trained on how to complete the screening process, and indicators that warrant additional information needed for admissions into ATR programming. Procedure All Together Recovery Admissions Staff provide information about services, answer questions to potential referral sources and/or clients, and collect pertinent clinical information to screen for appropriateness of admission to treatment. Individuals may present to All Together Recovery for direct admission, having been referred by a healthcare professional (physician, nurse, therapist, etc.), State Health Professional Programs, churches, pastors, employers, licensure boards, etc. If clients present directly to treatment at All Together Recovery facilities the intake screening process will be completed in person rather than over the phone. The following types of information are collected to begin the intake process: ● Identifying information. ● Types of substances used and patterns of use. ● Current withdrawal symptoms, and date of last use and amount used at that time. ● Current psychiatric status, prior diagnoses, and/ or any relevant medication/ treatment history. ● Current physical status, and ability to complete ADLS. ● Precipitating situations/factors prompting admission. ● Status of support systems and distress. ● Presence or absence of legal, vocational, financial, or other areas of functioning that might impact the individual’s participation in treatment. ● Medical history, including current and history of any chronic or acute medical conditions, medications, and ongoing treatments. ● Prescription medication list with doses and directions. ● Self-harm, SI/HI history and risk of current thoughts ● Trauma and abuse history. ● Criminal and legal history ● Referral source ● Insurance information ● Medicare/Medicaid status Every individual requesting admission to All Together Recovery services will first undergo an initial pre-screening with a member of the ATR Admissions Staff. The following indicators require further approval from clinical and or medical staff before progressing to the next stage of admission.
