About

Four Rivers Behavioral Health is a Community Mental Health Center. The Wm. H. Fuller Center is located at 1525 Cuba Rd. in Mayfield, KY. We provide both outpatient, intensive outpatient and Residential Substance treatment as well as outpatient Mental Health Services. Our Residential program is licensed for 18 beds and we provide non-medical detox as well as 28 days of substance abuse treatment with both clinical and 12 step programming. We also offer Crisis Services on the same campus.

Referral / Admission

  • Admission criteria
    • We utilize the ASAM criteria for substance abuse placement criteria. We accept both males and females. The assessment is determine that a person meets criteria for this level of care.
  • Referral process
    • Call to schedule an Initial Assessment, if seeking residential substance abuse treatment or traditional outpatient services, walk in emergencies are acceptable for outpatient services. We also offer a weekly substance abuse group and an Intensive Outpatient Substance Abuse Program.
