As part of the Hazelden Betty-Ford Foundation Patient Care Network, KentuckyCare provides Medication Assisted Treatment that utilizes the COR-12 ( Comprehensive Opioid Response with Twelve Steps) to help individuals find their path to recovery. Our interdisciplinary team (medical provider, nurses, therapist, recovery coaches, behavioral health coordinator) works closely with the patient to recommend the most appropriate pathway to treatment and recovery.

Referral / Admission
- Admission criteria
- The patient will need to meet with the Behavioral Health Clinician at KentuckyCare (270-575-3247) or Four Rivers Behavioral Health (270-442-8039) to complete an assessment and to inform of the program requirements. Once the assessment is complete the case will be presented to treatment team and a determination of services will be developed. The patient must be committed to attended the appropriate level of care treatment and support groups.
