The Wellspring Bernie Block Wellness Center offers programs to serve adults diagnosed with serious mental illnesses and are based upon the principles of recovery. Case Managers/Therapists assess clients’ needs and develop person-centered plans to assist clients in pursuing their individual recovery goals.

- Individuals are encouraged to take all medications provided by their treatment provider. Wellspring staff do not handle patient medication.

Referral / Admission
- Referral process
- Clients are referred to the program by Community Housing staff, CSU staff, community treatment providers, hospital staff and other professionals. Individuals and/or family members may self-refer for Case Management or Therapeutic services as well.
- Admission criteria
- • Have a diagnosis of serious mental illness as defined by the Department of Behavioral Health • Have Medicaid or Medicare insurance or the ability to pay a private fee established annually by administrative staff • Be 18 years of age or older While the primary mission of the program is to support individuals with severe and persistent mental illness diagnoses in living in the community, individuals with co-occurring physical health, substance abuse, intellectual disability, or acquired brain injury are also eligible for outpatient and case management services.
