Referral / Admission

  • Admission criteria
    • Journey 2 Heal does not discriminate against clientele needing assistance. We are open to all individuals, and have the ability to refer elsewhere if higher level of care or specialization, is required.
  • Referral process
    • To refer clients to our organization, the referral source can faxed over the clients basic information to (606) 780-0038. We require full name, date of birth, social security number, address, and insurance information. If the client is uncomfortable providing that information, you may have them call us at (606) 780-0115 to provide the information needed to get them verified and into our system to ensure care.
