The Commitment House provides substance abuse treatment for adults 18 and over. The Commitment House’s Mission: Our mission is to provide a person–centered program that instills the tools to maintain long-term sobriety; by changing the way people live and think. The Commitment House’s Vision: To create a program where treatment and recovery work together to produce a better outcome. _________________________________________________________________________________________ The Commitment House was established on November 1, 2011, as a 6-month transitional living program for men. Today, The Commitment House has grown into an AODE and BHSO licensed, CARF and BBB accredited, full-spectrum drug & alcohol addiction treatment program for men and women with over 90 employees who work hard every day to provide quality service. _________________________________________________________________________________________ Person-Centered Treatment Options _________________________________________________________________________________________ Holistic, Person-Centered Treatment Our clients are adults with poly-substance abuse disorder who may also have chronic or urgent medical and mental health disorders. We strive to provide, non-judgmental, non-biased, trauma-informed care for our clients. Healthy Life Training Clients receive classes on disease management, nutrition, and how to take care of themselves and live life without drugs and alcohol. Relapse Prevention Planning Clients take an active role in creating their life in recovery through treatment and relapse prevention planning. Detox with Residential Treatment We provide medical withdrawal management and comfort medications for detoxification. Residential Treatment lasts 28-30 days and provides a time to heal, learn about addiction and gain the tools to maintain long-term sobriety. Intensive Outpatient & Outpatient Program IOP and OP include clinically facilitated groups and sessions each week in a community living environment, in addition to the following services: - IOP includes 12 hours of groups - OP includes 3 hours of groups - 12-Step Meetings and Sponsorships - Peer Support Specialists - Case Management - Employment Opportunities - GED Program PICC Line Accessible We are equipped to provide substance abuse treatment for those with congestive heart failure and addiction. Here are suggested items to bring to residential treatment: Personal Item List All items must be unopened and CANNOT contain alcohol in the first three ingredients. -- up to 5 days’ worth of clothes -- Shower shoes -- Laundry detergent* -- Personal hygiene items* -- Twin sheets, blanket, and pillow* -- Non-perishable snacks like cookies or crackers (you will not have access to a refrigerator, but you may use a microwave) -- Cigarettes/Dip -- Money for vending machines -- Medium-sized lock for locker -- Prescription medication in correct containers (subject to approval and assessment by our medical team) -- Release papers from jail (if applicable) *We can provide these items if you do not have them.

- All clients receive a full medical assessment by The Commitment House physician to assess needs. All medications to bring in are reviewed by medical team to assess whether necessary and appropriate.

Referral / Admission
- Admission criteria
- Call (270) 900-0373 for screening. The screening phone call takes about 20 minutes, and most decisions are made within an hour.
- Referral process
- Client will need to see their primary case manager and they will contact us for an opening. We also accept referrals from courts.
